How to Get Services

Our supports are geared toward the philosophy of person-centered thinking. The individual is at the center of decisions made about the services they receive. We’re committed to helping people realize their dreams!

Eligibility is required at four stages of life. Read below to learn more about our supports and when eligibility is required.

Early Intervention (birth until age 3)
Our goal is to give parents, families, and children the tools they need to succeed. In conjunction with your Help-Me-Grow service and support administrator, we’ll assess your infant or child for early intervention and guide you through the eligibility process. Our staff will talk to you about your challenges and goals.

Eligibility for Early Intervention (birth until age 3)
If you live in Butler County, Ohio, you can begin the eligibility process for babies from birth until their 3rd birthday by calling Help-Me-Grow at (513) 785-6850.

Preschool Age (3-5)
We’re here to support you and your child. At age 3, your child may qualify for an Individualized Education Plan through their school, which spells out exactly what services the child is to receive. We will determine your child’s eligibility and can assign a service and support administrator to help navigate the system.

Eligibility for 3 through 5 years old
To receive services, your child must reside in Butler County, Ohio and have documentation of at least two developmental delays or an established risk.

School age services
We will determine eligibility for your child, and can assign a service and support administrator. Your child may qualify for an Individualized Education Plan through their school, which spells out exactly what services the child is to receive. The IEP includes annual goals (both academic and functional) and benchmarks or short-term objectives. Your child’s Support Coordinator will connect you with valuable resources, answer questions, and give support based on yearly goals. We’ll connect you with adult-transition resources, job opportunities, and more.

Eligibility for 6 through 15
To receive services, your child must reside in Butler County, Ohio, have a verified developmental disability and at least three limitations noted on the COEDI assessment tool. The COEDI assesses a child’s mobility, self-care, self-direction, capacity for independent living, learning, and receptive and expressive language. Children will have their eligibility re-determined at age 16 for continued services.

Eligibility for 16 and older
To receive services, the individual must reside in Butler County, Ohio, have a verified developmental disability, and at least three limitations noted on the OEDI Assessment Tool. The onset of the disability must be documented prior to the age of 22 and must be likely to continue indefinitely. The OEDI assesses an individual’s mobility, self-care, self-direction, capacity for independent living, learning, receptive and expressive language, and economic self-sufficiency.

We will help you determine eligibility and can assign you with a service and support administrator who will connect you with valuable resources, answer questions, and give support based on yearly personal goals. We can connect you with job opportunities, help find a place to live and connect you with great opportunities.
